一、電話預(yù)約： 撥打24小時(shí)電話預(yù)約（0757-22220999）。

二、現(xiàn)場預(yù)約：

（一）門診人工窗口掛號（8:00~12:00；14:00~17:00）。

（二）自助機(jī)。

（三）醫(yī)生診間。

（四）關(guān)注“南方醫(yī)科大學(xué)順德醫(yī)院”微信小程序掛號。

電話預(yù)約流程 

Make an appointment by phone

1、撥打24小時(shí)電話預(yù)約（0757-22220999）

1.Call 24-hour appointment line for registration (0757-22220999).

2、提供有效證件(港澳通行證/護(hù)照/社?？?、就診卡

2.Provide valid identity documents (Hong Kong and Macau permit / passport / social security card) and medical card.

3、預(yù)約所需醫(yī)生、就診日期、就診時(shí)間

3.Confirm the doctor, consultation date and time.

4、預(yù)約成功短信發(fā)送到手機(jī)

4.The notification of the registration will be sent to your phone.

5、就診當(dāng)天憑有效證件在門診人工掛號窗口取號

5.Get the number at the manual registration window of outpatient department with a valid ID on the day of treatment.

6、成功取號后，即可前往相應(yīng)科室候診、就診

Make an appointment on-site

1、持港澳通行證/護(hù)照/社?？ㄔ陂T診人工窗口辦理就診卡，并現(xiàn)場掛號。

1.Apply for a medical card at the outpatient manual window with your Hong Kong and Macau permit / passport / social security card, and register on site.

Make an appointment on-line

1、掃碼關(guān)注“南方醫(yī)科大學(xué)順德醫(yī)院”小程序。

1.Please scan the QR code to follow the WeChat mini program of “Shunde Hospital of Southern Medical University”.

2、點(diǎn)擊“個(gè)人中心”，手機(jī)號碼/微信登陸后，選擇就診人管理，立即添加。

2.Enter “Personal Center”, log in to your account with your cell phone number/WeChat, and add patients’ information to the patients management.

3、點(diǎn)擊“沒有身份證”，完成個(gè)人信息登記，上傳證件24小時(shí)內(nèi)等待后臺(tái)認(rèn)證。

3.Choose “No ID” to complete the registration of personal information and upload the valid ID for authentication within 24 hours.

4、已通過認(rèn)證的首次就診人士請致電0757-22220999人工建檔后，獲取電子就診卡再掛號。

4. For new patients who have passed the authentication, please call 0757-22220999 to manually set up a file and get an electronic medical card before registration.

5已認(rèn)證并有本院就診卡，可選擇掛號預(yù)約，依次選擇預(yù)約科室、預(yù)約日期、預(yù)約醫(yī)生，確認(rèn)就診時(shí)間并15分鐘之內(nèi)繳費(fèi)(超時(shí)號源將自動(dòng)取消)。

5.Patients who have passed the authentication and have our medical card can directly click the “Registered” to select the department, appointment date and doctor in order, confirm the appointment time and complete the payment within 15 minutes (the registration will be cancelled automatically if payment is not made after the time limit).

預(yù)約成功后，就診當(dāng)天取號流程

The procedure for taking the number on the day of treatment after a successful appointment 

1、電話預(yù)約的憑有效證件(港澳通行證/護(hù)照/社保卡)在門診人工收費(fèi)窗口取號。

1.Those who make appointment by phone can take the number at the outpatient manual window with a valid ID (Hong Kong and Macau permit/passport/social security card).

2、線上預(yù)約的進(jìn)入醫(yī)院范圍后，打開“南方醫(yī)科大學(xué)順德醫(yī)院”小程序選擇“就診報(bào)到”，或憑小程序新建的二維碼到各樓層自助機(jī)取號。

2.when people with online appointment arrive at the hospital, please open the WeChat mini program of “Shunde Hospital of Southern Medical University” to click “check in” or use the QR code generated by the program to take the number from the self-service machine on each floor.

3、成功取號后，即可前往相應(yīng)科室就診，就診過程中有任何問題可至護(hù)士臺(tái)站咨詢。
